The registration form that patients need to fill in is ideally before their consultation, please ensure we emphasize that they need to fill in front AND back of the sheet.

Procedure of Booking the Patient
Over the phone/in store customer:

• Book customers in with a tele-health clinical pharmacist via google calendar at specific available times.
• Confirm with the patient that they are a tele-health consultation and they don’t need to come into the pharmacy for the consultation.
• Book customers in for 30 min consultation. Get the patient’s full name and contact number.
• Take a pre-payment of $25 and note down in the calendar that customer has paid.
• The customer is contacted the night before their consultation to confirm their booking.
• Tele-health pharmacists contact customers at the time of their appointment.

The process of booking online is done via Bookly on the St Francis Medical Centre. Once the customer books online, it shows up on our google calendar where the customer gets contacted to confirm the appointment and make the pre-payment of $25.

Policy
VACCINATION PROCEDURE FOR:

• Influenza (Patients 10 years and over)
• MMR, dTpa  & Meningococcal ACWY (Patients 16 years and over)

Description
St Francis Group has guidelines regarding vaccinations done by St Francis Pharmacists. 

Criteria for vaccination

1. The Pharmacist vaccinator must have successfully completed an approved vaccination course training and have a current First Aid and CPR certificate. Another staff member with current first aid and CPR certificate must be in store at the time of vaccination.
2. Our consult room must always be equipped with an in date Anaphylactic reaction kit.
3. Written consent must be documented before each vaccination by patient or parent/guardian using the Guild immunisation checklist.
4. Details of vaccination must be recorded into AIR.
5. Adverse reactions must be reported to WAVSS and the patient’s GP.

Scope
The following guidelines are to be adhered to on a business unit level

Procedure

1. Patient books an appointment at a time when the vaccinator is rostered on with another staff member– Appointment is noted on daily handover sheet and booking calendar.
2. When patient arrives, they are welcomed by name, asked to pay for vaccine and are given a Guild immunisation checklist to fill out. The receipt should be attached to this form to notify vaccinator that they’ve paid
3. Pharmacist are then to lead them to the consult room; check that they are eligible to receive the vaccine and have signed the consent form.
4. Once vaccinated, the patient must be observed for 15 minutes 
5. Details of their vaccination must be entered into AIR.
